If you are 24 and under and live in Bedfordshire or Hertfordshire you can request a chlamydia home testing kit by filling in the form below. Packaging is plain and discreet. All you have to do is provide a urine sample and post it back. Both the kit and postage are free so testing for chlamydia couldn’t be easier!
Please note that kits can take up to 10 days to arrive.

If you are 25 and over you are not eligible to access the screening programme but you can still get tested - if you are sexually active it is important to get yourself checked out. Just click here to find out where your nearest GUM clinic (Genito-Urinary Medicine) is and make an appointment. This service is free and totally confidential.

NOTE: Please remember to label both the green form and the small UPT bottle otherwise the sample will not be tested.
